Residential Habilitation Planning Coordinator

Aspire of WNY is looking for a Fulltime Residential Habilitation Planning Coordinator who is responsible for integrating, coordinating and monitoring each resident’s habilitation services. Primary responsibilities include oversight of Staff Action Plan and related requirements and Individual Plan of Protection including sign off of plans as reviewer, auditing EMR for compliance, ensuring that billing can be supported by documented services and overall management of the delivery and supervision of residential habilitation services. Ability to use the Electronic Medical Record (EMR) regarding assigned duties, knowledge of Therap are preferred.

Minimum Qualifications:

• At least 1 year of experience working directly with persons with intellectual disabilities or other developmental disabilities
• Is either a licensed medical professional, or a human services professional possessing at least a Bachelor’s degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, psychology or recreation therapies
• Valid NYS Driver’s License

Hourly rate of pay: $20.94 – $26.40
