Qualifications:

Registered Nurse with a current N.Y.S. license. One year of professional clinical nursing experience is required.

Primary Responsibility:

Ensures through providing medical service that individuals served have opportunities to make choices, develop and achieve, participate in the community, maintain good health and secure personal future, be treated with respect and dignity and experience relationships with family members, friends and neighbors.

Essential Physical Functions are the ability to:

Critical and crucial to the position’s essential function is the safety of the people ASPIRE serves. Due to the nature of the individuals we serve, the essential functions require all the duties and responsibilities listed below to be performed in all environments and using all appropriate equipment.

Essential Physical Functions are the ability to:

  1. Bend, stoop, squat, kneel, sit, stand and lift as per program needs.
  2. Provide two-way communications with people.
  3. Transfer, move, reposition, shift, and place people in pieces of equipment necessary for their physical circumstance ensuring their safety, comfort, and their natural living conditions as per program needs.
  4. Physical agility/ability to react to a lift-threatening emergency or to intervene in a crisis situation including using your body as an intervention (SCIP or other techniques), as prescribed, to assure safety of the individuals. Physical agility/ability includes but is not limited to bending, stooping, squatting, kneeling, sitting, standing and lifting as per program needs.

Duties and Responsibilities:

  1. Functions as a member of the Interdisciplinary Team and participates in the development of Individual Program Plans.
  2. Responsible for nursing care assessments, treatment planning and provision of health care assessment, restorative and/or supportive care.
  3. Completes or provides input into nutritional assessments; dietary counseling and diet control measures as indicated.
  4. Executes medical regimen prescribed by a licensed or otherwise legally authorized physician, physician’s assistant, nurse practitioner or dentist.
  5. Administers medications as prescribed; monitor effects of medication

and the overall regimen.

  1. Responsible for the control of communicable diseases and infections through identification and assessment, reporting of communicable diseases and infection in accordance with law, and implementation of appropriate protective and preventive measures.
  2. Ensures that the on going medical needs of individuals are adequately met and maintained (e.g. on-call responsibilities).
  3. Administers immunizations and maintenance of such records.
  4. Coordinates and manages individual’s medical needs, planning and linking with other health care providers.
  5. Provides first aid as needed.
  6. Records medical information and coordinates health care records with other health care providers.
  7. Provides individual and group education in preventative health measures and health maintenance.
  8. Provides staff training on health issues and provides consultation as assigned.
  9. Conducts family health education as needed.
  10. Coordinates support nursing personnel activities so as to enure

continuity of care for individuals served.

  1. Inventories and maintains supply of drugs, medical supplies and equipment.
  2. Participates in maintaining a safe work environment.
  3. Participates in program and community committees as assigned.
  4. Participates in staff training, in-services and meetings as required.
  5. Adheres to Agency’s policies, procedures, rules and regulations.

21.     Performs other duties as assigned.
