High school diploma. Ability to stand, sit, kneel, stoop, lift, assist in physical care and therapeutically handle individuals. Valid New York State driver’s license, reliable transportation suitable for transporting program participant, certification in Med Administration, SCIP and First Aid, as required by program needs,.

Primary Responsibility:

To provide direct support services to the people supported by the Agency, and to provide a continuum of community based learning opportunities to the participants of the Agency and to ensure that individuals served have opportunities to make choices, develop and achieve, participate in the community, maintain good health and secure personal future, be treated with respect and dignity and experience relationships.

Assists the participant(s) with the management of their daily services and with the development of training programs and provides community based instruction to people receiving services; assist with the provision of personal care as needed; provide maintenance and assistance with learning opportunities; provide personalized training.

Critical and crucial to the position’s essential function is the safety of the people Aspire serves. Due to the nature of the individuals we serve, the essential functions require all the duties and responsibilities listed below to be performed in all environments and using all appropriate equipment.

Essential Physical Functions are the ability to:

  1. Bend, stoop, squat, kneel, sit, stand and lift.
  2. Provide two-way communications with people.
  3. Lift and transfer the individual both manually and mechanically.       Manual lifting requires the ability to lift individuals as required in their prescribed individual program plans—bearing the weight of that individual. Mechanical lifting will be performed using proper mechanical lifting standards and based on the safety of the individual.
  4. Transfer, move, reposition, shift and place people in pieces of equipment necessary for their physical circumstance ensuring their safety, comfort and their natural living conditions.
  5. Physical agility/ability to react to a life-threatening emergency or to intervene in a crisis situation including using your body as an intervention (SCIP or other techniques), as prescribed, to assure safety of the individuals. Physical agility/ability includes but is not limited to bending, stooping, squatting, kneeling, sitting, standing and lifting.

Duties and Responsibilities:

  1. Functions as a member of the participant’s Circle of Support and participates in the development of Individual Service Plans, and Day Hab Plans.
  2. Implements Individual Service Plans including documentation of interventions. Includes implementations of service plans on an individual and group basis.
  3. Provides instruction, assists in the development of curricula and provides one-on-one and small group instruction to individuals in accordance with their daily activities.
  4. Completes documentation, attendance records, skill assessments, incident reports, medication documentation, event reports, or daily service documentation as assigned.
  5. Provides training and assistance in and performs as indicated routine program activities, therapy carry over activities and direct support services i.e. personal care needs, meal preparation, shopping, recreation activities, housekeeping, as well as maintenance according to Individual Service Plan, active treatment schedule and/or need.
  6. Transports, accompanies and assists individuals in community based learning, day program activities and community training using their personal vehicles, agency vehicles when available or public transportation; as required.
  7. Administers medication and performs related duties as assigned.
  8. Ensures integration of services and acts as a communication linkage through contact with families and/or care takers, correspondents, staff members, transporters and other service providers. Includes informing the designated supervisor(s) of information vital to service operation and care of individuals served.
  9. Teaches participants personal care skills, and works alongside to perform routine stock or inventory work, periodic repair work, organization of functions including but not limited to laundry, cleaning/maintenance of adaptive equipment, household and other service areas.
  10. Responsible for the supervision of individuals who attend the program at all times, based on personal safeguard requirements.
  11. Monitor for unsafe conditions and practices in public locations or in the individual’s day program situation to ensure health, safety and well-being of individual served.
  12. Coordinates positive relationships and communication among individuals served, families, advocates, involved others, Circle of Support and the Agency.
  13. Participates in in-services, staff training, committees and meetings as required.
  14. Acts as an appropriate role model for the individuals served.
  15. Assists in the training of new employees as assigned by supervisor.
  16. Adheres to Agency’s policies, procedures, rules and regulation.       As a condition of employment, adherence to attendance and lateness standards is required.
