Turning Disabilities into Capabilities
A multi-disciplinary team approach is used when planning for a student’s transition, which can begin as early as age 12. The plans should become part of the student’s Individual Education Plan (IEP) and include such adult outcomes as:
- Meaningful employment, continuing education or day services
- Age appropriate leisure activities
- Accessible community housing
- Accessible transportation
Aspire offers the following services to assist with the coordination of transition planning and the implementation of these plans:
- Individual Transition Plan (ITP) Development
- Vocational Assessment
- Technology Services
- Job Development and Training
- Social and Recreational Activities

The multi-disciplinary team approach includes a combination of the following programs and services:
Aspire coordinates services that the student’s transition team identifies as needed to support the student and the Transition Plan. Builds relationship over time so the student has the opportunity to have the same service coordinator as an adult.
Aspire's Center for Learning provides transition services to students who have multiple disabilities through:
- Assisting the student’s team (parents, teachers, therapists, social workers, etc.) with transition planning that is part of the student’s IEP
- Informing families about the process and their role
- Linking and referring families to services
- Providing students with work training opportunities
- Providing a unique summer school option
Individualized Career & Transition Services include the following for students transitioning from high school to adult life:
- Person-centered vocational profiles
- Vocational experiences to fulfill IEP goals
- Consultation on transition and employment goals for IEP vocational curriculum

Aspire's Technology Services mean that successful transition can be achieved when students are able to retain and use assistive technology devices and services without interruption during the transition period. Services include assessment, training, and high school equivalency and adult basic education (GED/ABE).
