School to Adult Life Transition

A multi-disciplinary team approach is used when planning for a student's transition, which can begin as early as age 12. The plans should become part of the student's Individual Education Plan (IEP) and include such adult outcomes as:

Aspire coordinates services that the student's transition team identifies as needed to support the student and the Transition Plan. Builds relationship over time so the student has the opportunity to have the same service coordinator as an adult.
