  • Overview

    As experts in providing health care for individuals with complex disabilities, Aspire provides comprehensive primary and specialty care services in a caring environment using a patient-centered focus.

    The Aspire on WNY Health Center, located at 7 Community Drive in Cheektowaga, New York, offers a full range of services delivered by integrated clinical teams.

    • Patients receive a comprehensive health assessment outlining past problems and current health concerns.
    • Nurses provide total care management in a supportive effort to improve and maintain patient wellness.
    • Neurologists and Psychiatrists are on staff providing thorough medical evaluations to target specific health issues.
    • Physical Therapy, Occupational Therapy, Speech Therapy and Behavioral Health are offered on site by an experienced therapy team who work in concert with primary care providers and other specialists to develop comprehensive care plans.

    Aspire of WNY’s experience and collaborations with local health care agencies and medical providers, and connectivity to important regional technology providers such as HEALTHLink™, support our clinical environment in which services and resources are accessed quickly and seamlessly. Aspire’s newly implemented Patent Portal, provides patients and caregivers access to important health records keeping them up to date on diagnostic testing results and annual preventive requirements.

  • Primary Health Care

    Providing integrated healthcare in an environment promoting the dignity and individual differences of each patent is the core focus of Aspire’s primary, specialty, and therapeutic care services.

    The Aspire Health Center Primary Care Team provides coordination of services through a clinical team of physicians, nurse practitioners, nursing staff, and patient representatives. This collaborative effort insures a healthcare experience that is seamless, convenient, and patient-centered. At Aspire, the complex needs of each patient are considered at every visit, whether acute or routine, with a focus that supports the patient’s care plan as it relates to wellness, functional health, and emotional well-being.

    Currently, Primary Care is provided Monday through Friday, 8am to 4pm.

    Continuity of patient care is assured through 24 hour on-call service and beginning in 2015, convenience care hours will be offered one evening per week and Saturday mornings, to better serve patient needs.

  • Assistive Technology Network

    Our multi-disciplinary team conducts evaluations for alternate access to traditional forms of technology, as well as enhanced functioning within traditional living and workplace environments. Adaptations can make battery operated and electronic devices accessible for environmental control, vocational pursuits, community integration and enhanced self reliance. Specialized equipment and environmental modifications facilitate independent living and vocational functioning.

    The ATN Team:

    Aspire has been providing assistive technology services for more than 20 years. The Assistive Technology field is a specialization or niche service only recently considered by other agencies. Aspire’s ATN team includes a broad spectrum of professionals from the following disciplines:

    Assistive Technology:

    Computer Access

    ATN specializes in providing people of all abilities with access and training to use computers. We customize computer systems using individualized hardware and software modifications for people interested in gaining computer skills.

    Switches & Adaptive Toys

    ATN makes play and learning accessible to children through the combination of toys and technology. Our switches and adaptive toys allow for multi-sensory appeal, self-expression, adjustability and safety.

    Seating & Mobility

    ATN custom builds or adapts existing equipment to meet specific needs and provide just the right amount of support and security. Our extensive expertise with mobility devices such as wheelchairs, standers, gait trainers and bicycles brings greater independence and mobility.

    Adaptive Equipment

    ATN custom designs and modifies a wide variety of equipment to increase mobility, communication and independent living. Equipment includes standers, layers, floor chairs, bathroom aids, beds, bikes, sleds, tables, pointers and grabbers.

    Augmentative Communication Clinic

    ATN offers evaluation and training in the use of assistive communication aids and devices for individuals who have lost the ability to speak or whose speech is difficult to understand. Our thorough assessment of an individual’s language skills, needs and potential ensures they are fitted with the most appropriate and effective communication equipment.

  • Mental Health Services

    As part of Aspire’s Health services, the Mental Health Services Department has been providing individuals with behavioral health care since 1971.

    Our mental health clinicians, under the direction of nationally renowned psychiatrist, Dr. Thomas Hays, have significant expertise in the diagnosis and treatment of mental health conditions common in individuals with developmental and other complex disabilities.

    Oftentimes, the individuals we serve have difficulty expressing their feelings and wishes due to a variety of reasons, including communication barriers, cognitive impairments, or challenges in the world around them. Our clinical staff understands how to listen and communicate effectively while offering evidence-based interventions, in spite of any existing communication barriers.

    Our assessment and treatment services are designed to help people cope with, or overcome, life’s traumas and emotional, or other mental health challenges. Our goal is to help those we serve strive toward their highest level of capabilities and personal fulfillment.

    Psychiatric Services:

    • Evaluation, treatment, and medication monitoring are provided

    Psychology Services:

    • Behavioral counseling, programming, and testing are provided.
    • Psychological assessments are performed, including IQ tests, adaptive skills, academic skills, and screening for psychiatric problems and developmental disabilities.

    Counseling Services:

    • Individual, family, and group counseling is available.

  • Rehabilitation Services

    Physical Therapy

    In order to maximize functioning, reduce pain and promote good health, our Physical Therapists conduct evaluation and develop individualized treatment programs to prevent, minimize, or alleviate physical disability and associated health complications. Evaluations and training related to custom seating and mobility systems are additional specialty areas. Patient education and carry-over home exercise programs allow individuals take an active role in their health care and recovery.

    Occupational Therapy

    Promoting independence and productivity in all areas of daily living, Occupational Therapists provide evaluations and treatment related to sensory dysfunction, activities of daily living, functional arm and hand skills, bathing skills and environmental awareness. Specialized services include hand orthotics and adaptive equipment to enhance the capacity to participate in all areas of life. Individualized treatment plans are designed to maximize independent functioning and are customized in accordance with a person’s skills, needs and interests.

    Speech Therapy

    Speech-Language Pathologists provide evaluation and treatment of a full range of communication disorders and swallowing difficulties. Qualified clinicians address all issues affecting speech, language, and comprehension. Augmentative and Alternative Communication systems offer a ‘voice’ to those who have lost the ability to speak or are difficult to understand. Our dedicated clinicians embrace the unique potential in every person served, with a focus on the fact that enhanced communication skills are a key factor in facilitating a sense of empowerment and satisfaction in life.

    Aspire’s health team also offers the following:

    • Wheelchair evaluations, ordering and fitting.
    • Orthotic services to help prevent further contractures and deformity, enhance ambulation, and maintain optimal functioning.
    • Adaptive equipment evaluation and ordering of supplies though Occupational Therapy.
    • Occupational, Speech, Physical Therapies.

  • Specialty Health Services

    Neurology

    A neurologist who specializes in developmental disabilities and effective seizure management is on staff to diagnose and treat these disorders.

    Podiatry

    A podiatrist is on staff to deal with problems of the feet.

    Physiatry/Orthotics

    Evaluations are performed to ascertain the best functional ability of the individual using physical therapy, adaptive equipment and orthotics (bracing). Services are also available in the Southern Tier.

    Feeding and Swallowing

    Trained Speech Pathologists and Occupational Therapists evaluate an individual’s ability to self feed and to swallow food and liquid safely. Recommendations are made based upon the individual’s current skill level and potential for advancement. Adaptive meal-time equipment can enhance independence during meals. Modified diet textures can make food and liquid easier to manage. Compensatory techniques and meal time strategies can enhance swallowing safety and improve respiratory function. Formal intervention plans are implemented to maximize an individual feeding and swallowing skills.

Communication aids and devices, along with equipment to facilitate alternate access, are available on-site to facilitate hands-on trials and training.

    Seating and Mobility

    Physical Therapists and Occupational Therapists evaluate for custom seating systems that manage medical symptoms and provide therapeutic levels of support and security for individuals with orthopedic, postural and mobility challenges. Devices such as wheelchairs, standers, gait trainers, stair lifts and elevating seats assist individuals in maneuvering independently throughout their environment.

    Women’s Health

    An experienced provider is on staff to offer routine women’s gynecological care.
